Client Forms
Use this form for any new client with one (1) animal (you can also submit it multiple times if you have multiple animals, it is just easier to use the other form if you have more than one (1)!
New Client Form (Multiple Animals)
Use this form for any new client with more than one animal that will be seen by Western States Equine Veterinary Services. Existing clients can also use this form to update information as needed.
Appointment Forms
Health Certificate / Coggins (EIA) Testing Form
Use this form when needing a health certificate and/or Coggins (Equine Infectious Anemia) testing. Failure to complete this form accurately and promptly will result in delay of certificates as well as possible additional charges.
Pre-Purchase Form (Buyer)
Use this form when requesting a pre-purchase. Both the buyer and seller agreements need to be on file prior to the pre-purchase.
Pre-Purchase Form (Seller)
Have the seller complete this form prior to the pre-purchase. If needed, the buyer can complete this form.
Reproductive (Mare) Services Form
Use this form when starting reproductive services (breeding soundness exams, annual breeding contracts, etc.).
